Uniform Certification Application
The Uniform Certification Application is accepted for DBE certification by the Illinois Department of Transportation, City of Chicago, Chicago Transit Authority, Metra and Pace. In accordance with the IL UCP, you need only to apply to one of the participating agencies listed above.

The attached application has been developed to determine whether your firm is eligible for Pace's DBE program and to provide information regarding the capability of your firm in your designated area of expertise. In order to be eligible as a DBE, your firm must meet the eligibility standards established by the U.S. Department of Transportation (USDOT) regulation 49 CFR part 26.
